Privacy verklaring

UW PRIVACY

Voor een goede behandeling is het noodzakelijk dat ik, als uw behandelend homeopaat, een dossier aanleg. Dit is ook een wettelijke plicht opgelegd door de WGBO. Uw dossier bevat aantekeningen over uw gezondheidstoestand en gegevens over de uitgevoerde onderzoeken en behandelingen.

Ook worden in het dossier gegevens opgenomen die voor uw behandeling noodzakelijk zijn en die ik, na uw expliciete toestemming, heb opgevraagd bij een andere zorgverlener bijvoorbeeld bij de huisarts.

Wij doen ons best om uw privacy te waarborgen. Dit betekent onder meer dat wij:

• zorgvuldig omgaan met uw persoonlijke en medische gegevens,

• er voor zorgen dat onbevoegden geen toegang hebben tot uw gegevens

Als uw behandelend homeopaat heb ik als enige toegang tot de gegevens in uw dossier. 

Ik heb een wettelijke geheimhoudingsplicht (beroepsgeheim).

De gegevens uit uw dossier kunnen ook nog voor de volgende doelen gebruikt worden:

• Om andere zorgverleners te informeren, bijvoorbeeld als de behandeling is afgerond of bij een verwijzing naar een andere behandelaar.  Dit gebeurt alleen met uw expliciete toestemming.

• Voor het gebruik voor waarneming, tijdens mijn afwezigheid.

• voor het geanonimiseerde gebruik tijdens intercollegiale toetsing.

• Een klein deel van de gegevens uit uw dossier wordt gebruikt voor de financiële administratie, zodat ik of mijn administrateur, een factuur kan opstellen.

Als ik vanwege een andere reden gebruik wil maken van uw gegevens, dan zal ik u eerst informeren en expliciet uw toestemming vragen.

Deze gegevens in het cliëntendossier blijven zoals volgens de wet op de behandelovereenkomst is vereist 20 jaar bewaard.

PRIVACY OP DE ZORGNOTA

Op de zorgnota die u ontvangt staan de gegevens die door de zorgverzekeraar gevraagd worden, zodat u deze nota kan declareren bij uw zorgverzekeraar. 

• uw naam, adres en woonplaats 

• uw relatienummer 

• uw geboortedatum

• de datum van de behandeling

• een korte omschrijving van de behandeling, zoals bijvoorbeeld ‘behandeling natuurgeneeskunde’, of 'psychosociaal consult’

• de kosten van het consult

YOUR PRIVACY

For proper treatment, it is necessary that I, as your treating homeopath, create a file. This is also a legal obligation imposed by the WGBO. Your file contains notes about your state of health and data about the examinations and treatments performed.

The file also contains data that are necessary for your treatment and that I, after your explicit consent, have requested from another healthcare provider, for example from your general practitioner.

We do our best to guarantee your privacy. This means, among other things, that we:

handle your personal and medical data with care,

ensure that unauthorized persons do not have access to your data

As your treating homeopath, I am the only one who has access to the data in your file.

I have a legal duty of confidentiality (professional secrecy).

The data from your file can also be used for the following purposes:

To inform other healthcare providers, for example when the treatment has been completed or when referring to another practitioner. This only happens with your explicit consent. For use for observation, during my absence.

for anonymized use during peer review.

A small part of the data from your file is used for the financial administration, so that I or my administrator can draw up an invoice.

If I want to use your data for another reason, I will first inform you and explicitly ask for your permission.

This data in the client file will be kept for 20 years, as required by the law on the treatment agreement.

PRIVACY ON THE HEALTH CARE BILL

The health care bill that you receive contains the data requested by the health insurer, so that you can declare this bill with your health insurer.

your relationship number

your date of birth

the date of the treatment

a brief description of the treatment, such as 'natural medicine treatment', or 'psychosocial consultation'

the costs of the consultation
