Privacy statement (English)
For proper care it is necessary that I, as your homeopath, keep a client file. This is also a legal obligation under the WGBO. Your file contains notes about your health and details of the consultations and advice. It also contains information that is necessary for your care and that I requested, with your explicit consent, from another healthcare provider such as your GP.
My commitment
- I handle your personal and medical data with care.
- I make sure unauthorised persons have no access to your data.
- I am the only person with access to your file, and I have a legal duty of confidentiality (professional secrecy).
Use of your data
Data from your file may also be used:
- to inform other healthcare providers (for example on referral), only with your explicit consent;
- by a locum during my absence;
- in anonymised form for peer review;
- to a small extent for the financial administration, so that I or my accountant can prepare an invoice.
If I want to use your data for any other purpose, I will first inform you and ask your explicit permission.
Retention period
In line with the law on medical treatment agreements, your file is kept for 20 years.
Privacy on your invoice
Your invoice contains the details your health insurer requires so you can claim it:
- your name, address and place of residence
- your customer (relation) number
- your date of birth
- the date of the consultation
- a short description, such as "classical homeopathy consultation"
- the cost of the consultation
Privacyverklaring (Nederlands)
- om andere zorgverleners te informeren, bijvoorbeeld bij verwijzing, alleen met uw expliciete toestemming;
- door een waarnemer tijdens mijn afwezigheid;
- geanonimiseerd tijdens intercollegiale toetsing;
- uw relatienummer
- uw geboortedatum
- de datum van het consult
- een korte omschrijving, zoals "consult klassieke homeopathie"
